Notice of Availability of Form 1095-C

Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, is available upon request.

Employees may request a copy of their Form 1095-C by contacting the Human Resources/Benefits Office at kassysuggs@frankstonisd.net or 903-876-5920. A copy will be provided no later than January 31, 2026, or within 30 days of the date the request is received, whichever is later.

The IRS does not require Form 1095-C to be submitted with your federal income tax return, but it should be kept for your records.
